Care management refers to the overseeing and coordinating of all of the care a child and their family receives to help them navigate the child’s health services. Care managers assess family needs and connects them with the appropriate resources. As a result, care managers help ensure that families and their children are receiving all of the resources and support for which they are eligible. This care may include everything from physical and occupational therapy to medication management, counseling, and community-based resources. Care managers also ensure that children who struggle with mental health challenges are receiving the support they need to thrive.
